LFBA Parent and Player Surveys
How did we do?
Thank you for participating in the LFBA program this summer. We hope you had a great experience.
Every year, we strive to make improvements to the program, and we feel your evaluations and comments are critical in our efforts. Please take the time to complete a Parent or Player survey. The survey should take no longer than 5 minutes to complete.
Your answers and comments will be anonymous. (How? The survey itself is hosted at a third party survey company that stores all the information and provides LFBA only aggregated data. There’s simply no way for anyone within the LFBA community to track who fills out the survey. Unless, of course, you put your name on it!) However, if you prefer to fill out a paper form and mail it in, please see the bottom of this page.
We ask that you fill out only the appropriate survey only once. After you’ve completed the survey, you will not be able to access it a second time.
Surveys will no longer be accepted after September 1.
